LightSpring Home Care ensures the highest standards for the selection and hiring of Certified Nursing Assistants and Home Health Aides. We also understand that each patient faces unique circumstances and has individual needs, and therefore need to be carefully matched with appropriate caregivers. Our professionals are supervised and trained by our Client Services and Skilled Nursing team.

In-Home Care Plans tailored to Patient Needs

Before the start of care, each patient receives an intake visit from our Client Services Manager who gathers all the relevant details from the patient and the patient’s loved ones. This includes relevant medical history, specific clinical needs and other preferences detailed by the patient and their loved ones. This process forms the basis for the customized care plan that will be created for the patient. The goal is to support the patient in their own home for as long as possible. To achieve this our caregivers will provide a range of services to support activities of daily living such as bathing, dressing and grooming, medication reminders, light housekeeping, and more.

Patients With Specialized Needs

LightSpring Home Care serves patients from a range of backgrounds and with a variety of special needs including the ongoing management of various chronic conditions. Our caregivers are able to support patients following hospital or rehabilitation center stays, as well as patients requiring specialized care as they cope with specific conditions such as Alzheimer’s, memory loss, and other chronic conditions such as COPD, diabetes, stroke, arthritis, and Parkinson’s disease. As necessary, our caregiving team with partner with the patients medical care providers to ensure that long-term care needs are properly addressed.

Working with Patients’ Families and Communities

We believe that quality care can only be achieved in partnership with the patients’ loved ones. This means that LightSpring Home Care will make every effort to understand our patients’ specific circumstances to ensure that quality care is provided. Our staffing team will work with families or other community members to arrange as many ‘meet and greets’ as is needed to achieve the right caregiver to patient fit. Patients and their families will have access to and communicate with a LightSpring Home Care care coordinator who is specifically assigned to their case.
