LightSpring Home Care is committed to increasing opportunities for older Pennsylvanians and individuals with physical disabilities to remain in their homes.

If you’re 21 or older and have both Medicare and Medicaid, or receive long-term supports through Medicaid because you need help with everyday personal tasks, you’ll be covered by Community HealthChoices.

We welcome family-member caregivers through the PA Family Caregiver Support Program. Our caregivers are paid on a weekly basis and are offered a range of benefits as part of their full-time employment.

About Community HealthChoices

The Department of Human Services (DHS) is committed to increasing opportunities for older Pennsylvanians and individuals with physical disabilities to remain in their homes. If you’re 21 or older and have both Medicare and Medicaid, or receive long-term supports through Medicaid because you need help with everyday personal tasks, you will be covered by Community HealthChoices (CHC).

CHC will coordinate your health care coverage to improve the quality of your health care experience — serving more people in communities rather than in facilities, giving them the opportunity to work, spend more time with their families, and experience an overall better quality of life.

Care provided by our home health aide may include some of the following:

  • Assisting the client with Feeding/Eating
  • Transportation Arrangements to Medical Appointments
  • Preparing Meals (According to Client’s Dietary Specifications)
  • General Monitoring of Health Conditions
  • Help the Client with Daily Personal Tasks (Bathing and Dressing)
  • AM and PM Care
  • Linen Changes and Laundry
  • Bathing Assistance
  • Provide assistance with client personal care, grooming and toileting.
  • Provide assistance with mobility and ambulation.
  • Prepare meals and assist in feeding and/or providing fluids as needed
  • Provide relief for bed-ridden clients such as positioning, massaging and skin care.
  • Perform range of motion exercises.
  • Reminders for self-administration of medication.

Each client is assigned a care coordinator who is responsible for all aspects of care and can be reached each day.​

We have an on-call team available after hours and weekends to deal with emergency coverage issues.​

We have a skilled nurse who makes rounds, and trains CGs based on specific needs of individual clients.
